Membership Application

The Membership Application Form is a Word Template that you can download to your computer and complete, using Microsoft Word. The template contains grayed areas that are fields. Click once in the field and you will be able to type your information. For the areas that require placement of an “X”, click twice in the field and a dialog box – Text Form Field Options will open; change the default value from “not checked” to “checked” and click ok.

Print the document and mail it along with a check made payable to:
